On this page, you will find information about what is needed to allow for us to provide to your students health needs. If you have a question or would like to speak to the nurse about your student’s health needs, please reach us at the numbers listed above.

COVID-19 Information: We are dedicated to following the CDPHE and JPH guidelines on addressing COVID-19. Temperature and symptoms screenings are performed on each student who enters the building. We are asking families who have a positive in the household or have someone in the household getting tested for COVID, or has a known exposure to COVID, to please keep your child home and speak to the district RN to figure out quarantine times. Also, if your child has any of the following symptoms, please keep them home and get testing if not resolved within 48 hours:
Fever, chills or fatigue/Loss of taste or smell/Shortness of Breath/Muscle or body aches/Cough or sore throat/Runny nose or congestion/Nausea, vomiting or diarrhea/Headache

If you child does get tested for COVID, please notify district RN of date of testing and results.

Preventing the spread of Illness: During these challenging times, we recommend preventing the spread of illness by washing hands when able, using hand sanitizer, avoid touching your face, not sharing drinks, coughing/sneezing into the elbow, remaining home when sick, and maintaining six feet apart when possible. As always, being up to date on your current recommended vaccines is key to preventing communicable diseases. Please speak to your primary care provider to ensure all vaccinations are up to date.  

Medications at the School: Included below are forms needed for students to have medications at the school. Please review them carefully. All forms must be signed by a physician and returned to the school nurse or health aide. Please read the following before bringing medications to the school:

  •  All prescription medications must have the students name, an expiration date, medication name, dosage, instructions, and the physicians name on it or the school cannot accept the medication. A medication agreement form will be needed unless your child has asthma, allergies, or a seizure plan.
  • All over the counter medications need to be labeled with the students name, must be in the original packaging, and must match the exact name and dosage listed on the medication agreement form. Please note: Brand medications (such as Tylenol) will be accepted if the physician wrote Acetaminophen (the generic version of tylenol) on the medication agreement form. Different medications, such as Ibuprofen instead of tylenol, will not be accepted. A medication agreement form will be needed unless your child has asthma, allergies, or a seizure plan.
  • Any controlled substances will need to be counted in and out with the parent and follow the same requirements as prescription medications. A medication agreement form will be needed unless your child has asthma, allergies, or a seizure plan.
  • If your child has an asthma, allergy, or seizure plan the medication form will not be required as the medication is written on said plans. All plans require physician signature.
  • Any expired medication will be denied.
  • All medications must be picked up by parents at the end of the school year or medications will be disposed of.
